We will work closely with you to create a personalized treatment plan based on your specific needs. All of our services are performed at our physical therapy clinic in South Farmingdale, NY for your safety and convenience. What to Expect from Your First Appointment:
Your initial evaluation will take approximately one hour.

Please arrive 15 minutes early to your first appointment at our Long Island physical therapy clinic. You will need to complete new patient forms, including a medical history questionnaire. Bring your photo ID, insurance card, comfortable clothing, and any prescriptions from your treating physician. Co-pay, co-insurance and deductible payments are due at the time of service.

Insurances Accepted:
We accept most PPO plans and Medicare. Please call us at 516-665-0882 for details or to check your specific benefits and coverage. We also offer special cash rates.

How to Get to Our South Farmingdale, NY Clinic


When you choose to work with our Long Island clinic, you’ll enjoy convenient access to our full range of physiotherapy and performance services. We are located just south of Bethpage Black Course and west of Republic Airport. If you head northeast on NY-135 N and take exit 7E, you will continue down the road for approximately 1 mile. Make a right turn onto Fulton Street and then another right onto Heisser Lane. When you arrive at Evolution PT, it will be on the left-hand side.
