What do I need to know before my first physical therapy appointment?
Your initial evaluation will take approximately one hour.
Please arrive 15 minutes early for your first visit to complete the necessary paperwork. Filling our all of your paperwork prior to your first appointment will speed up the check-in process. If you wish to do this, please click on and print the relevant paperwork HERE. Please be sure to download the 'New Patient Form' as well as the paperwork specific to the area we will be treating.
On your first visit to physical therapy, please bring:
Your Driver's License or identification card
Your insurance card
Prescription from your treating physician (if applicable)
Expect subsequent treatments to last approximately one hour.
Co-pay, co-insurance and deductible payments are due at the time service is provided.
PLEASE NOTE: We enforce a strict 24-hour-notice cancellation policy.
Do I need a prescription for physical therapy?
Nope! As doctors of physical therapy, we are established as the first point of contact for all matters relating to the musculoskeletal system.
What insurances do you accept?
We accept most PPO plans and Medicare. Please call us for details or to check your specific benefits and coverage. We also offer special cash rates.
Where do I park?
PLAYA VISTA LOCATIONS: have designated parking spaces at the PT building at 11825 MAJOR ST, or in the designated lot located at the corner of Major St. and Mesmer Ave. There is also side street parking available. There is no cost to park in our designated lots.
BEVERLY HILLS LOCATION: has side street and metered parking surrounding the building.
BRENTWOOD LOCATION: has a parking lot at the building.
SOUTH BAY LOCATION: has free parking at the building.
SANTA MONICA LOCATION: has free parking behind the building.
COSTA MESA: has free parking at the building.
DARIEN, GREENWICH and STAMFORD LOCATIONS: have free parking at the building.

PT circuit classes
What makes the PT classes unique?
The PT classes are all taught by licensed physical therapists who are experts in proper movement to provide you with explicit direction, help you achieve your goals safely and effectively and help you return to exercise post-treatment.
Class sizes are limited to give you individualized care and all of the components of a complete wellness plan are emphasized in these classes: cardiovascular conditioning, flexibility, strengthening, stabilization, balance and proprioception.
There are three PT classes that we offer: (1) core strengthening, (2) upper body strengthening and (3) lower body strengthening. They are all eligible for insurance reimbursement. These classes are a great way to get back into working out and maintaining health after injury once physical therapy treatment is complete. MORE DETAILS
How can I get my insurance company to pay for PT classes?
First, you must have a PPO insurance plan and complete a physical therapy evaluation. This evaluation will identify any weaknesses, tightness, instability or any other dysfunction that will be relayed to the therapist leading class. This will give you the most individualized attention to address your particular body's needs. REQUEST AN APPOINTMENT
PLEASE NOTE: ALL INSURANCE COVERAGE IS DIFFERENT AND THEREFORE, THE EXACT AMOUNT THAT EACH PARTICULAR PLAN WILL COVER VARIES. IT IS IMPORTANT TO VERIFY YOUR PLAN WITH YOUR INSURANCE COMPANY PRIOR TO YOUR CLASS OR TREATMENT.
How much do I pay if my insurance company is covering a PT class?
Each individual's insurance benefits are different. You will only be responsible for paying your deductible or co-pay/co-insurance, which is due at the time of service. If you don't know what the specifics of your physical benefits are, you can call your insurance company or contact us and we will provide a courtesy verification to get as detailed information on your personal benefits and coverage as we can.
Is there anything else I need to do prior to coming to a PT class with insurance?
Yes, you must first SCHEDULE AN APPOINTMENT for a one-on-one evaluation with a physical therapist who will do a comprehensive orthopedic assessment and give you a home exercise program to address your impairments. Your physical therapist will determine if a PT class is appropriate for you, and if so, will recommend which class will be most beneficial. Your PT will also educate you on proper form and mechanics that we will be looking at and adjusting while you are taking the group PT class.
Can I just pay cash for PT classes?
Yes! And many of our patients who have graduated from their physical therapy care do continue with us in these classes and with personal training. Patients paying cash for PT classes are not required to complete the one-on-one evaluation with a therapist in advance.
Is Evolution Fitness a gym?
Nope! Evolution Fitness is an advanced training, performance and recovery center where we provide skilled and detailed instruction with group exercise classes. The center was founded and is facilitated by physical therapists who are experts in biomechanical movement and exercise. Our instructors and trainers are very meticulous with proper form and technique with all classes in order to provide you with the most effective and efficient workout while being safe and eliminating the risk of injury. How do I sign up for classes or book recovery equipment and training sessions?
You can see the complete schedule of classes HERE. To book, you must create an account. Once you are logged in, you can sign up for classes and purchase class or personal training packages and Alter G, recovery boots, massage and pilates sessions. Class packages can be completely purchased and signed up for online. You may purchase a membership or class package through Triib HERE. Personal training, Alter G, recovery boots, massage and pilates sessions can all be purchased online, but please REQUEST AN APPOINTMENT or CONTACT US to check availability and book your session.
What is the Alter G?
The Alter G-Trainer is an anti-gravity treadmill. It is a state-of-the-art machine which enables users to reduce their effective body weight during exercise. The anti-gravity technology behind the Alter G was originally developed by NASA. Air pressure is used to give the user a feeling of being unweighted, allowing them to set their effective body weight between 20% and 100% in 0.1% increments. It allows you to run without pain, train through injuries and supercharge your workouts. What are recovery boots?
Recovery boots offer full foot, ankle and leg compression therapy that uses sequential, dynamic air compression for massage therapy, improved circulation and faster recovery post-exercise. Can I drop into a class without booking through Triib online?
Yes! You pay pop into a class and pay at the front desk prior to the class. However, we cannot 100% guarantee that space will be available as clients reserving their spot online do have priority. Your first fitness class is FREE!
Do I need to bring anything to a class?
Please bring your own water to class. If you forget, we have bottles available for purchase at select locations. We suggest bringing a towel as well.
Do you have showers?
We have shower facilities available at Evolution Physical Therapy Culver City/Playa Vista located on the 3rd floor at: 11825 Major St. - just next door to Evolution Fitness. The showers are accessible Mon - Fri 7AM - 7PM and Sat 9AM - 2PM.
For physical therapy evaluations, physical therapy treatments*, PT classes and personal training sessions:
24 hour notice is required for cancellations. Please cancel by calling the appropriate CLINIC. If you cancel within 24 hours or don't show up, you will be charged a $40 cancellation fee. This fee will be charged to the credit card on file. (*Physical therapy treatments include: physical therapy, massage, pilates and yoga sessions.)
For group fitness classes:
Because classes are limited in size, two hour notice is required for cancellations. Please cancel through Triib or by calling the appropriate CLINIC. If you cancel within two hours of the start of class or don't show up, you will be charged the full amount of the class. This will be charged to the credit card on file.
Can I purchase "evolve" merchandise?
Yes! We sell a variety of workout/athleisure tops, bottoms and accessories with our evolve logo, as well as foam rollers, lacrosse balls and other PT and fitness equipment. Please check out our retail areas; select merchandise available at our Playa Vista, Beverly Hills and South Bay locations.
